Reservist to Active Duty

  1. Fill out a withdrawal form
  2. Fill out a petition to backdate
  3. The student needs to assign a person to be involved in student’s school affairs, they need to fill out the Optional Authorization to Release Student Education Record
    • A FERPA release still needs to be filled out, even if the spouse or family has a Power Of Attorney.
  4. Bring the withdrawal form, petition to backdate, and a copy of orders to the Registrar’s office.
    • If a FERPA release is to be turned in, turn in with all forms to the Registrar’s office.
  5. Notify Veteran’s Financial Aid to terminate claim for the semester.
  6. Notify Fronske, if the individual has heath insurance through school. A partial refund may be available for the remaining months.
  7. Notify Parking Services, a partial refund may be available (Contact Student Veterans Center).
  8. Storage Units are available in town, if personal belonging need to be stored.

  9. Resident Life needs to be notified so that the Residence Hall Contract can be released.
    • The Front Desk of the dorm can provide a Residence Hall Contract Release. It can be turned back into the front desk of the dorm. The RA will get the paper work started. Within 24 hours of withdrawal you must contact your hall staff to officially check out of your room.

The Uniformed Services Employment and Reemployment Rights Act of 1994 (USERRA, 38 U.S.C. §§ 4301 – 4335) is a federal law intended to ensure that persons who serve or have served in the Armed Forces, Reserves, National Guard or other “uniformed services:” (1) are not disadvantaged in their civilian careers because of their service; (2) are promptly reemployed in their civilian jobs upon their return from duty; and (3) are not discriminated against in employment based on past, present, or future military service. The federal government is to be a “model employer” under USERRA. See 38 U.S.C. § 4301.
