SSW DEPARTMENT TRAVEL

TRAVEL PROCEDURES

1. Complete and sign the following forms at least three weeks prior to travel:
   • RECORD OF ABSENCE
   • APPLICATION FOR TRAVEL FUNDING
2. Forward the completed forms to Kooros Mahmoudi for his signature.
3. Upon travel completion, provide the following to Hilary McDonald within FIVE DAYS:
• TRAVEL REIMBURSEMENT CHECKLIST
• ALL NECESSARY RECEIPTS
4. Hilary McDonald will prepare the Exhibit J document and forward it to you for your signature.
5. If you have any questions, please contact Hilary.

NOTE: 

• REMEMBER to print at least two airline quotes in addition to the documentation for the flight that you book. Be sure to complete your booking at least 21 DAYS PRIOR to travel. Airline quotes need to be from two DIFFERENT websites (Expedia, Travelocity, etc.).
• If flying with family members, please book your flight separately or get documentation from the airline for your portion of the cost of the flight.

OUTSIDE FUNDING

In addition to Department funds, some funding may be available to you through the SBS Dean's Office (for international trips) or through the Provost's Office (for domestic travel). Applicants should apply in advance by the Oct. 1 and Feb 8th deadlines. Contact Hilary McDonald for more information.
