FORMS

Unless otherwise noted, all forms are compatible with both Microsoft Word and Adobe Acrobat format. If you do not have Microsoft Word, then go to the Adobe web site and download Acrobat Reader for free.

Please note that not all forms are designed to be completed from the browser screen, but rather, completed after they are printed. Many form downloads are in two pages. The first page contains the instructions and the second page is the form.

Mac users:

First you must have Adobe Reader installed on your Mac. You can download Adobe Reader for free. When you download Adobe Reader for the first time, a plugin named AdobePDFViewer.plugin should be installed as well. This plugin is installed in the Library/Internet Plug-Ins folder on your Mac.

Check to verify that the AdobePDFCViewer.plugin is installed in the Library/Internet Plug-Ins folder on your Mac. If it is not installed, you can download this plugin separately, for free, at http://www.caddpower.com/cms/files/AdobePDFViewer.plugin.zip. Select that you want to save the file. Save the file to your desktop, then unzip the AdobePDFViewer.plugin and place it in your Library/Internet Plug-Ins folder on your Mac.

Once you have Adober Reader installed and this plugin installed in your Library/Internet Plug-Ins folder you need to have Adobe set up as your default PDF viewer. Launch Adobe, go to Preferences > Internet and check the “Display PDF in Browser” option.

Save your preferences when finished. Safari is now configured to allow you to view PDFs in Adobe instead of in Safari’s native PDF viewer.

Residency Petition Dates /Deadlines Spring 2010 Term

First day to submit residency petition - Monday, October 26, 2009
Deadline to submit residency petition - Friday, January 22, 2010
Deadline to file reclassification appeal (if residency petition was denied) - Friday, February 26, 2010

Change to Residency Status Undergraduate Forms
Arizona Domicile Affidavit (Spring 2010) Designated for students financially dependent on parents who are domiciled in Arizona.
Change Residency Status Petition (Spring 2010) Designated for independent students
Military Petition to Change Residency Status (Spring 2010) Designated for members of the Armed Forces of the United States
Non-Resident Teacher Waiver (Spring 2010)
Spouse Domiciled in Arizona (Spring 2010) Designated for students who have married an Arizona resident
Native Indian Residency Verification (Spring 2010) Designated for Native American Indian students who are domiciled within Indian Reservation borders of Arizona and whose borders extend into another state

Address Change Log onto LOUIE and navigate to Personal Portfolio > Addresses.

Administrative Drop/Add Form
Enrollment (Add)/Drop Request For immediate processing, log on to LOUIE and add/drop classes in self-service.
Override Authorization Complete for class capacity, time conflict, requisites for course, department or instructor consent.
Override Authorization - Audit/Class Links/Out-of-Career Complete this form if you are doing one of the following: Auditing a class. If you want to enroll in one component of a class, i.e., just a lecture or just a lab. If you are an undergraduate student enrolling in a 500/600 level course, or if you are a graduate student enrolling in a 400 level course for credit.

Request to Waive Late Enrollment/Petition Fees
Unit Load Override Approval To carry an overload of more than 19 hours for undergraduate students or 16 hours for graduate students during the fall and spring terms. Overload information for summer and winter sessions can be found on those respective web sites.
Withdrawal from NAU Complete this form when you are dropping all classes during any given term.
