Note: Registration forms will not be accepted until the noted registration dates.

Mountain Jacks Kid's Camp

• NAU Continuing Students (must be enrolled for classes summer or fall 2009, provide class print out)
• Current NAU Employees
• Current NAU Rec Center Members
• Community (limited spaces available)

* Priority Registration for NAU Students: Begins Friday, March 13, 2009
* Open Registration for NAU Community: Begins Friday, April 10, 2009

Dolphin Day Camp

There are NO eligibility requirements. The program is open to the Flagstaff community.

Louie's Leaders

There are NO eligibility requirements. The program is open to the Flagstaff community.

Fee Structure

If you are registering for... Summer Session I only.

• Payment is due in full at time of registration.

If you are registering for...Summer Session II only.

• Before July 2, 2009 - a non-refundable $75.00 deposit is due at time of registration. The deposit holds a place for your child and is applied toward the total registration fee. Balance is due on July 2nd.
• After July 2, 2009 - payment is due in full at time of registration.

If you are registering for...Summer Session I & II.

• Payment for SSI is due in full at time of registration. SSII fees are due on July 2nd. When registering for both sessions, payment of SSI holds your childs place for SSII.

Financial Assistance

NAU Students can contact the Office of Student Life to apply for the Childcare Voucher Program. The voucher program reimburses parents at their award level of between 20-50% per week per child, depending on demonstrated need.

Those registering for Dolphin Day Camp and Louie's Leaders, can apply for the Lemon Foundation Scholarship. This need based scholarship can cover up to 100% of registration costs. Contact Terry Huntley at the Wall Aquatic Center, 928-523-6324, for more information.

Registration Process

1. Download the registration packet - select the appropriate camp below.
• Mountain Jack's Kids Camp - Pre-K Only (Ages 3-4)
• K-6th Registration Form (Dolphin Day Camp & Mountain Jack's)
• Louie's Leaders (Ages 12-15)
2. Read all program policies & procedures.
3. Complete packet and return the following (one complete packet per child) to the SYP Office:
• Registration Form
• Code of Conduct (initialed & signed)
• Program Card (signed!)
• Shot Records (current)
• School Schedule (NAU students only!)
4. Return completed packet to the NAU Rec Center - either drop off or mail in, PLEASE DO NOT FAX!
