IRS Form W-9S is only required for the following purposes:

Providing a Student's Social Security Number (or ITIN):

If NAU does not have your Social Security Number (SSN) or Individual Taxpayer Identification Number (ITIN) then they are required by the IRS to provide this number to Northern Arizona University for IRS reporting purposes.


This information is protected by the Family Education Rights and Privacy Act (FERPA) as all other student academic and financial information is protected.

(Note: Failure to provide a correct SSN or ITIN to NAU without reasonable cause is subject to a $50 fine by the IRS)
